This is a one-time foster and you expect to be adopting your dog, and just a temporary member of ESRA.

If you now own or have previously owned other dogs, please give a brief history of each, mentioning names, genders, and breeds. Please also indicate spay/neuter status of each dog.
Example: Had Ivey, spayed female poodle, from 2002 - 2014. Euthanized at old age. Currently have a 5 year old neutered male black lab. Have had him since he was a puppy.
Do you own ANY other animals? In addition to cats, also include birds, reptiles, rodents, rabbits. Describe species and ages and where in the household these animals live.
Do you know if your current dog prefers the company of same sex/opposite sex or older/younger dogs?
Do you know your current dog(s)' play style (i.e. vocal and on the sidelines, physical body slammers, wrestlers, chasers and tuggers, tentative soft touch players)?
Training and Behavior
Are you aware of the various issues (i.e. competitiveness, resource guarding for food, toys, resting spots, water, caretakers) that can lead to conflict and/or aggressive behaviors in multi-dog homes?

Could you care for a Special Needs Springer (one with mobility limits, vision limits, or medication needs)?

We will review history for a number of years. If you have used multiple vets, please list them ALL. If the account(s) may be under someone else's name, please provide that. (If you've never had a pet, you can enter N/A.)

One-time Fostering for ESRA
When the Code of Ethics is signed to cover a one time foster, or foster-to-adopt situation, I understand my membership in ESRA will automatically terminate as soon as this dog either leaves my home and I am no longer fostering, or is formally adopted by me, unless membership status is requested by also submitting the Membership Application.
This application is subject to further screening and approval to foster. Please proceed to the Code of Ethics to complete this application process.
